摘要:
目的 探讨外周血AngⅡ/Ang-(1-7)比值与多囊卵巢综合征(PCOS)患者的胰岛素抵(insulin resistance,IR)抗及脂代谢的关系。方法 选取2016年5月至2018年2月于我院妇产科就诊并确诊为PCOS的患者143例为研究组,选取同期于我院进行体检的健康人106例作为对照组,并根据IR指数将研究组分为非IR组(n=83)和IR组(n=60)。抽取受试者清晨空腹血清,检测两组受试对象的体质量指数(BMI)、空腹血糖、甘油三酯(TG)、高密度脂蛋白(HDL)和低密度脂蛋白(LDL)、血清卵泡雌激素(FSH)、促黄体生成素(LH)、睾酮(T)、外周血血管紧张素II(AngII)和血管紧张素-(1-7)[Ang-(1-7)],并将AngII、Ang-(1-7)及AngII/Ang-(1-7)与相关临床指标行Pearson相关性分析。结果 非IR和IR组患者AngII和Ang-(1-7)水平均高于对照组(P均<0.05),其中非IR组Ang-(1-7)水平高于IR组(P<0.05),IR组患者AngII/Ang-(1-7)比值均高于非IR组与对照组(P均<0.05)。IR组患者的BMI、TG、LDL值均高于非IR组与对照组(P均<0.05),HDL、LH值均低于非IR组与对照组(P均<0.05),IR组与非IR组患者T值均高于对照组(P均<0.05)。PCOS患者外周血AngII水平与LH呈负相关(P<0.05);外周血Ang-(1-7)水平与HOMA-IR、BMI、TG、LDL呈负相关(P均<0.05),与HDL呈正相关(P<0.05);外周血AngII/Ang-(1-7)比值与HDL及LH呈负相关(P均<0.05),与HOMA-IR、BMI、TG、LDL呈正相关(P均<0.05)。结论 PCOS患者外周血AngII、Ang-(1-7)处于较高水平,AngII/Ang-(1-7)比值失衡与PCOS的IR和脂代谢紊乱存在相关性。
Abstract:
Objective To investigate the relationship between Ang II/Ang-(1-7) ratio in peripheral blood and insulin resistance and lipid metabolism in patients with polycystic ovary syndrome(PCOS).Methods A total of 143 patients with PCOS who admitted in obstetrics and gynecology department of our hospital from May 2016 to February 2018 were selected as the study group.106 healthy people who underwent physical examination in our hospital at the same time were taken as the control group. According to the insulin resistance index, the study group was divided into non-insulin resistance group(n=83) and insulin resistance group(n=60).The fasting serum of the subjects was collected in the morning, the body mass index(BMI),fasting blood glucose, triglyceride (TG),high density lipoprotein(HDL),low density lipoprotein(LDL),follicle stimulating hormone(FSH), luteinizing hormone (LH), testosterone (T), peripheral blood angiotensin II (AngII) and angiotensin - (1-7) [Ang-(1-7)]were measured in both groups. Pearson correlation analysis was used to analyze the correlation between AngII, Ang-(1-7) and AngII/Ang-(1-7) and related clinical indicators. Results Ang II and Ang-(1-7) level in non-insulin resistance group and insulin resistance group was significantly higher than that in control group P all<0.05. Ang-(1-7) level in non-insulin resistance group was significantly higher than that in insulin resistance group. AngII/Ang-(1-7) ratio in insulin resistance group was significantly higher than that in non-insulin resistance group and control group, and the difference was statistically significant(P all<0.05).The BMI, TG, LDL in insulin resistance group was significantly higher than those in non-insulin resistance group and control group. HDL and LH was significantly lower than those in non-insulin resistance group and control group. T value in insulin resistance group and non-insulin resistance group was significantly higher than those in control group, and the difference was statistically significant(P all<0.05).The level of AngII in peripheral blood of PCOS patients was negatively correlated with LH (P<0.05).Ang - (1-7) level in peripheral blood was negatively correlated with HOMA-IR, BMI, TG and LDL(P all<0.05),and positively correlated with HDL(P<0.05).The radio of AngII to Ang-(1-7) in peripheral blood was negatively correlated with HDL and LH(P all<0.05), and positively correlated with HOMA-IR, BMI, TG and LDL(P all<0.05).Conclusions AngII and Ang-(1-7) in peripheral blood of PCOS patients are at high levels, and the imbalance of Ang II/Ang-(1-7) ratio is correlated with IR and lipid metabolism disorders of PCOS.
