摘要:
目的 通过CT重建确定标准位置的椎弓根螺钉,对应测量其在X线片上的位置并总结其特征,以提高徒手置钉的准确率。方法 回顾分析2017年1月至2018年1月我院收治的48例椎体骨折术后要求取内固定螺钉的患者,骨折椎体均为单节段固定,其中骨折椎及上下邻近椎体均被椎弓根螺钉固定。所有椎弓根螺钉均行CT扫描,筛选出被椎弓根骨质包容均匀、良好的螺钉,测量螺钉在X线片的位置并总结其特征。结果 288枚螺钉中,CT显示完全在椎弓根内的螺钉274枚(95.13%),椎弓根螺钉位置为1级的螺钉9枚(3.13%),2级的螺钉4枚(1.39%),3级的螺钉1枚(0.35%);CT位置良好的0级椎弓根螺钉X线片特征:椎体矢状径为(26.21±0.22)mm,椎弓根螺钉钉尖到椎体前缘距离为(8.53±0.17)mm,椎弓根内缘到棘突距离为(12.74±0.24)mm,螺钉钉尖到棘突距离为(6.29±0.13)mm。结论 标准位置的螺钉在X线侧位片上椎弓根螺钉与椎体终板相平行并且钉尖到椎体前缘的距离为椎体矢状径的1/3;正位片上椎弓根螺钉钉尖落在椎弓根内缘与棘突的中点附近处。
Abstract:
Objective To determine the position of the thoracolumbar pedicle screw at the standard position by CT reconstruction, and to measure its position on the X-ray film and summarize its characteristics to improve the accuracy of the freehand nail placement. Methods A retrospective analysis of 48 patients with thoracolumbar vertebral fractures admitted to our hospital from January 2017 to January 2018 was performed with internal fixation screws. The fractured vertebral bodies were fixed in single segment, including fractured vertebrae and upper and lower adjacent vertebral bodies are fixed by pedicle screws. CT scans were performed on all pedicle screws, and the screws with uniform and good vertebral bone mass were selected. The position of the screws in the X-ray films was measured and the characteristics were summarized. Results Pedicle screw misplacement were 9 screws in Grade 1 (3.13%),4 screws in grade 2 (1.39%),1 screw in grade 3 (0.35%). X-ray features of 274 pedicle screws in standard position :the average of sagittal diameter of vertebral body on lateral radiograph was (26.21±0.22) mm,the average distance from the tip of pedicle screw to the anterior edge of vertebral body was (8.53±0.17) mm, the average distance from the inner edge of pedicle to spinous process was (12.74±0.24) mm, and the average distance from the tip of pedicle screw to spinous process was(6.29±0.13) mm. Conclusion The screw of these standard positions was parallel to the vertebral endplate on the X-ray lateral slice and the distance from the tip of the nail to the anterior edge of the vertebral body was 1/3 of the sagittal diameter of the vertebral body. The anterior superior pedicle screw, the tip of the nail falls near the midpoint of the pedicle and the midpoint of the spinous process.
