摘要:
目的 探讨高血压合并冠心病患者血清中炎症因子的临床意义。方法 选取76例高血压合并冠心病患者、72例高血压患者、50例正常体检患者血清,采用ELISA方法检测不同组别血清炎症因子白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)的表达,分析其在不同组别中表达的相关性及与总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL)、高密度脂蛋白胆固醇(HDL)的相关性,并采用实时荧光定量PCR检测JAK和STAT的mRNA表达。结果 三组研究对象年龄、体质量指数(BMI)差异均无统计学意义(P均>0.05),三组患者的TC、TG、HDL-C、LDL-C,IL-1β、IL-6、TNF-α水平及JAK、STAT mRNA表达差异均具有统计学意义(P均<0.05),其中高血压合并冠心病组及单纯高血压组中TC、TG、LDL、IL-1β、IL-6、TNF-α及JAK、STAT mRNA的表达水平均高于对照组(P均<0.01),HDL低于对照组 (P<0.01);高血压合并冠心病组TG、IL-1β、IL-6、TNF-α及JAK、STAT mRNA高于单纯高血压组(P<0.05)。同时,IL-1β与IL-6、TNF-α表达和临床相关指标TC、TG、LDL均呈正相关(P均<0.01);与HDL呈负相关(P<0.01);TNF-α与临床相关指标TC、TG、LDL呈正相关(P均<0.01),与HDL呈负相关(P<0.01)。结论 炎症因子IL-1β、IL-6、TNF-α与高血压合并冠心病及临床相关指标TC、TG、LDL、HDL相关,这可能由JAK/STAT通路异常调控所致,可为高血压合并冠心病诊断及治疗提供一定的依据。
Abstract:
Objective To explore the clinical significance of serum inflammatory factors in patients with hypertension and coronary heart disease. Methods Total of 76 cases of hypertension with coronary heart disease,72 cases of hypertension and 50 cases of normal physical examination were selected. The expression of IL-1β,IL-6,TNF-α in serum of different groups was detected by ELISA. The correlation between the expression of IL-1β,IL-6,TNF -α in different groups and total cholesterol(TC), triglyceride(TG), low density lipoprotein cholesterol (LDL), high density lipoprotein cholesterol were analyzed The expression of JAK and stat mRNA was detected by real-time fluorescence quantitative PCR. Results There were no significant differences in age and BMI among all groups(P>0.05). However, the differences in TC,TG,HDL-C, LDL-C, IL-1β,IL-6,TNF-α,JAK STAT among the three groups were statistically significant(P<0.05). The TC, TG,LDL-C,IL-1β,IL-6,TNF-α,and JAK, STAT in hypertension group with coronary heart disease group were significantly higher than those in control group(P<0.01),and HDL-C was lower than the control group (P<0.01). TG,IL-1β,IL-6,TNF-α,and JAK, STAT in hypertension complicated with coronary heart disease were higher than those in patients with hypertension alone(P<0.05). In addition,IL-1β,IL-6 and TNF-αwere positively correlated with TC,TG and LDL-C(P all<0.01), but negatively correlated with HDL-C(P all<0.01). Conclusion The inflammatory factors IL-1β,IL-6 and TNF-αare associated with hypertension complicated with coronary heart disease and TC、TG、LDL-C、HDL-C,which may be regulated by JAK/STAT pathway. It may provide a basis for the diagnosis, prognosis and treatment of this disease.
