摘要:
目的 分析宁夏农村地区20~40岁居民体质指数(BMI)与代谢综合征(MS)及不同数目组分组合的关系,探讨BMI的截断值,为MS的防治工作提供依据。方法 以宁夏农村地区2008~2012年横断面调查为基础,选取年龄20~40岁自然人群为研究对象。根据MS诊断标准确诊的MS患者为MS组,1个代谢异常组分为H1组、2个代谢异常组分为H2组、3个代谢异常组分为H3组、4个代谢异常组分为H4组。采用偏相关分析研究BMI与血压、血糖(FBG)、血脂相关关系,非条件Logistic回归模型分析BMI与MS及不同数目组分组合的关系。使用受试者工作特征曲线(ROC)评估BMI的截断值。结果 H1、H2、H3和H4组的BMI、收缩压(SBP)、舒张压(DBP)、FBG、总胆固醇(TC)、甘油三酯(TG)呈现上升趋势,高密度脂蛋白(HDL-C)呈下降趋势,各组间的BMI、SBP、DBP、FBG、TC、TG、HDL-C的差异均有统计学意义(P均<0.05);随着BMI的升高,SBP、DBP、TG、TC、低密度脂蛋白(LDL-C)呈现上升趋势,HDL-C呈现下降趋势。与正常体重组比较,肥胖组的SBP、DBP、TG、TC、LDL-C升高(P均<0.05)。BMI与SBP、DBP、FBG、TG、TC、LDL-C呈正相关,与HDL-C呈负相关(P均<0.05)。BMI评估MS、H1组、H2组、H3组、H4组的截断值分别为24.53、19.07、23.18、23.92和23.62kg·m-2。随着BMI的增加,MS、H3组患病风险均呈上升趋势,超重组MS、H1、H2、H3和H4组患病风险分别是正常体重组的1.539、0.831、1.065、1.476和0.770倍。结论 20~40岁的宁夏农村人群中,体质指数BMI与代谢综合征、不同数目组分组合关系密切,BMI指标越高,MS及不同数目组分组合患病的风险越大;MS的BMI截断值稍高于国家推荐的界值点。
Abstract:
Objective The purpose of this study was to analyze the relationship between body mass index (BMI) and metabolic syndrome(MS),the combination of different number of components in Ningxia rural residents aged 20-40,and to explore the optimal cut points of BMI,and to provide evidence for the implementation of prevention and treatment of metabolic syndrome and component-related diseases. Methods Based on the cross-sectional survey of rural areas in Ningxia from 2008 to 2012,the natural population aged 20 to 40 was selected as the research object. MS patients diagnosed according to MS diagnostic criteria were MS group,1 metabolic abnormality group was H1 group,2 metabolic abnormality group was H2 group,3 metabolic abnormality group was H3 group,4 metabolic abnormality group was H4 group. The relationship between BMI and blood pressure,blood glucose and blood lipid was analyzed by partial correlation analysis, and the relationship between BMI and MS and the combination of different components was analyzed by unconditional logistic regression model. The receiver operating characteristic curve(ROC) was used to evaluate the cut-off value of BMI. Results BMI,SBP,DBP,FBG,TC,TG of H1,H2,H3 and H4 groups showed an upward trend, while HDL-C showed a downward trend. The differences of BMI, SBP, DBP, FBG, TC, TG, HDL-C among the groups were statistically significant (P<0.05);with the increase of BMI,SBP,DBP,TG,TC,LDL-C showed an upward trend, while HDL-C showed a downward trend. Compared with the normal weight group, SBP, DBP, TG, TC and LDL-C in obese group were increased (P<0.05). BMI was positively correlated with SBP, DBP, FBG, TG, TC and LDL-C,and negatively correlated with HDL-C(P all<0.05). The truncated values of MS, H1, H2, H3 and H4 were 24.53, 19.07, 23.18, 23.92 and 23.62 kg·m-2, respectively. The risk of MS,H1,H2,H3 and H4 was 1.539,0.831,1.065, 1.476 and 0.770 times higher than that of normal weight group,respectively. Conclusion BMI of rural population aged 20-40 in Ningxia is closely related to metabolic syndrome and composition of different numbers. The higher BMI, the greater the risk of MS and composition of different numbers of components. BMI cut-off value of MS is slightly higher than the national recommended threshold.
