摘要:
目的 应用实时三维超声心动图(real-time three-dimensional echocardiography,RT-3DE)评价房室传导阻滞患者右室流出道起搏前后左心房收缩功能的变化。方法 30例因II°和III°房室传导阻滞行右室流出道起搏的患者,在术前1周及术后1、3和6个月应用RT-3DE采集左心房全容积图像,获得左心房容积-时间曲线及左心房最大容积(LAVmax)、左心房最小容积(LAVmin)、左心房收缩前容积(LAVprep),通过计算获得左心房总射血分数(LATEF)、左心房主动射血分数(LAAEF)和左心房被动射血分数(LAPEF)。结果 与术前相比,术后1、3和6个月LAVmax差异均无统计学意义(P均>0.05),术后3、6个月的LAVmin、LAVprep降低(P<0.05);LATEF、LAAEF、LAPEF术后1个月与术前相比差异均无统计学意义(P均>0.05),术后3、6个月的LATEF、LAAEF、LAPEF均高于术前(P均<0.05)。结论 RT-3DE可以客观、准确地评价房室传导阻滞患者右室流出道起搏前后左心房收缩功能。
