摘要:
目的 研究下颌前移式矫治器治疗阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea hypopnea syndrome,OSAHS)患者,下颌逐步前伸对髁突表面所受Von Mises应力变化的影响。方法 运用锥形束CT、Mimics及Ansys软件建立OSAHS患者颞颌关节、下颌骨及上气道的三维有限元模型,模拟下颌骨逐步前伸,观测髁突表面各部位的Von Mises应力变化值。结果 随着下颌骨分别前移到最大前伸量的不同比例时,髁状突表面各部位所受的Von Mises应力也发生改变。当下颌骨前移量达到68%时,除髁突前斜面所受等效应力外,髁突其他部位受力大幅度减小,髁突前斜面的应力值变化最明显。结论 OSAHS患者戴用下颌前移式口腔矫治器治疗时,髁突前斜面与下颌前伸加载量密切相关。
Abstract:
Objective To explore the Von Mises stress of the condylar as the mandible gradually forward in patients with OSAHS.Methods The three-dimensional finite element model of the mandible and temporomandibular joint was established using CT, Mimics and Ansys software. The mandible forward was simulated and the change of the Von Mises stress of the condylar was observed.Results As the mandible gradually forward in different proportions of maximum protrusion, the Von Mises stress of the condylar had also undergone corresponding changes.When the mandibular forward amount to 68%,the Von Mises stress of condylar surface greatly reduced except the frontal surface. By statistical calculation, the Von Mises stress of the frontal surface of the condylar showed results with statistical significance.Conclusion When OSAHS patients use the mandible appliance device, the correlation of the mandible gradually forward and the Von Mises stress of condylar surface have different correlation and the most relevant of equivalent is the frontal surface.
