目的探讨晚期炎症因子高迁移率族蛋白B-1(highmobilitygroupbox1protein,HMGB1)在良性前列腺增生症(benignprostatichyperplasia,BPH)患者前列腺组织中的表达变化及其与BPH患者临床指标之间的相关性。方法收集40例行前列腺电切术的BPH患者前列腺组织标本,将其分为单纯BPH组和BPH伴慢性组织炎症组。HE染色法检测患者前列腺组织的病理情况,Westernblot法检测前列腺组织中HMGB1蛋白的表达,免疫组织化学检测HMGB1在前列腺组织中的分布;分析BPH患者前列腺组织HMGB1的表达与组织炎症、前列腺体积、患者年龄、血清前列腺特异性抗原(PSA)水平和前列腺症状(IPSS)评分等临床指标之间的相关性。结果40例BPH患者中52.5%(21例)伴有慢性组织炎症。与单纯BPH组相比,BPH伴慢性组织炎症组前列腺组织中HMGB1表达水平明显增高(P<0.01);HMGB1主要表达于前列腺上皮细胞和炎症细胞;单纯BPH组的前列腺组织中HMGB1主要分布于细胞核;BPH伴慢性组织炎症组HMGB1则可分布于胞核和胞浆。与单纯BPH组相比,BPH伴组织炎症组前列腺体积、患者年龄、血清PSA浓度和IPSS评分差异均无统计学意义(P>0.05)。BPH患者前列腺组织HMGB1蛋白表达水平与其前列腺体积呈正相关(P<0.05);BPH患者前列腺组织HMGB1蛋白表达水平与患者年龄、血清PSA浓度和IPSS评分无相关性(P>0.05)。结论晚期炎症因子HMGB1可能参与了BPH的发生过程。
Abstract:
Objective To investigate the expression of HMGB1 in prostate tissue of patients with BPH and its correlation with clinical parameters between patients with BPH. Methods The prostate tissues and clinical data of BPH patients were collected. HE staining was performed to examine the change of prostate histopathology in BPH patients. HMGB1 protein expression in prostate tissue of BPH patients was analysed by Western blotting. The expression of HMGB1 in the cell of prostate tissue in BPH patients was investigated with immunohistochemical technique. Correlation between HMGB1 protein expression in prostate tissue of BPH patients and clinical parameters of BPH patients, such as prostate volume, age, serum PSA levels and IPSS score were analyzed by using spearman correlation analysis. Results Prostatic inflammation existed in 21 BPH cases(52.5%)of 40 BPH cases. As compared to the simple BPH group, the expression of HMGB1 protein was significantly increased in BPH with chronic prostatitis group(P<0.01). However, the clinical parameters of patients, such as prostate volume, age, serum PSA levels and IPSS score were no significant difference between the BPH with chronic prostatitis group and simple BPH group(P>0.05). In simple BPH group, immunohistochemical staining indicated that HMGB1 mainly existed in the nucleus of prostate epithelial cells, and HMGB1 protein was secreted into the cytoplasm and extracellular in BPH with chronic prostatitis group. The levels of HMGB1 protein expression was positively correlated with prostate volume of BPH patients(P<0.05). There was no correlation between the levels of HMGB1 protein expression and age, serum PSA levels and IPSS score of BPH patients(P>0.05). Conclusion It can be concluded that HMGB1 may be involved in the development of BPH.
