摘要:
目的 探讨荧光原位杂交(FISH)法检测乳腺癌组织中HER2基因扩增的临床意义,并比较其与免疫组织化学(IHC)法检测HER2蛋白的一致性。方法 采用FISH法检测60例乳腺癌HER2基因扩增,并分析其与临床病理特征的关系; IHC法检测HER2蛋白表达,比较FISH和IHC检测HER2基因扩增与蛋白表达的一致性。结果 60例乳腺癌标本中,HER2基因扩增阳性率为45.0%,HER2基因扩增与患者年龄、肿瘤部位、肿瘤大小无关,与组织学分级有相关性(P<0.05)。IHC法检测HER2蛋白与FISH法检测HER2基因扩增的一致性较好(κ=0.633,P=0.000)。结论 HER2基因扩增与乳腺癌的恶性程度增高有关,IHC筛查HER2++和+++时需行FISH提高诊断的准确性。
Abstract:
Objective To observe the clinical significance of HER2 gene amplification in breast cancer using fluorescence in situ hybridization(FISH)method and to compare the consistency with immunohistochemistry(IHC). Methods HER2 oncogene amplification was detected using FISH method in 60 cases with breast cancer and the relationship with pathological parameters was analyzed. HER2 protein expression was detected using IHC method. The consistency of detection with two methods was compared and analyzed. Results In 60 cases with breast cancer, the positive expression rate of HER2 was 45.0%(27/60). HER2 gene had no correlation with patients' age, tumor site and tumor size while it had positive correlation with histological grade(P<0.05). Detection of HER2 by IHC had good consistence with FISH(κ=0.633, P<0.001). Conclusion HER2 is involved in the increase of malignant degree of breast cancer. FISH is required to improve the accuracy of diagnosis when HER2 ++和+++ screened by IHC.
