摘要:
目的 探讨外周血白细胞计数水平与代谢综合征之间的关系。方法 运用分层整群抽样的方法,选择2007年1月-2009年10月至宁夏医科大学总医院进行健康检查的宁夏地区政府机关及国有集体所有制企事业单位的职工作为调查对象,进行问卷调查、体格检查及实验室检测。结果 人群不同外周血白细胞计数组代谢综合征的患病率不同,随着外周血白细胞计数水平升高其腰围(WC)、体质指数(BMI)、收缩压(SBP)、舒张压(DBP)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白(LDL)、空腹血糖(FPG)及血尿酸(UA)水平增加(均P<0.05),高密度脂蛋白(HDL)水平降低(P<0.05)。Pearson相关分析结果显示,白细胞计数与WC、BMI、SBP、DBP、TC、TG、LDL、FBG及UA呈正相关(均P<0.01),与HDL呈负相关(P<0.01)。Logistic回归分析结果显示,在调整了年龄、性别、吸烟、饮酒、BMI、TC、LDL-C及UA后,白细胞计数的四分位组发生代谢综合征的危险性分别是白细胞计数最低组的1.779、1.575、1.443倍(均P<0.01),代谢综合征发病风险随白细胞计数的增加而增加。结论 外周血白细胞计数水平与代谢综合征密切相关,其可能成为代谢综合征的危险性预测指标。
Abstract:
Objective To explore the relationship between peripheral white blood cell count(WBC)and metabolic syndrome(MS). Methods With stratified cluster sampling method, the study population were selected from departments of government and enterprises and collective ownership of Ningxia. Data collection started in January 2007 and ended in October 2009. Questionnaire survey, physical examination, and laboratory test were conducted in participants. Results The prevalence of MS, waist circumference(WC), body mass index(BMI), systolic blood pressure(SBP), diastolic blood pressure(DBP), triglycerides(TG), total cholesterol(TC), low-density lipoprotein(LDL),fasting plasma glucose(FPG), serum uric acid(UA)significantly increased with increasing WBC(all P<0.05). In addition to the high-density lipoprotein(HDL),correlation analysis showed that WBC was positively related to WC, BMI, SBP, DBP, TC, TG, LDL, FBG and UA(all P<0.01). However, after adjusting for age, sex, smoking, drinking, BMI, TC, LDL and UA, the odds ratio(OR)for MS comparing quartile groups of WBC to lowest 1 was 1.779,1.575,1.443(all P<0.01),respectively. Conclusion The increase of WBC is positively correlated to MS. WBC may play a role in the pathogenesis of MS.
