摘要:
目的 探讨脉压、脉压指数与高血压合并2型糖尿病患者早期肾损害的相关性。方法 160例原发性高血压患者根据是否合并2型糖尿病分为:单纯高血压组(EH组)82例,高血压合并2型糖尿病组(EH+DM组)78例; 根据24h动态脉压(24hPP)水平将EH+DM患者分为:24hPP<60mmHg组(40例)和24hPP≥60mmHg组(38例); 根据24h脉压指数(PPI)将EH+DM患者分为 PPI<0.5 组(36例)和 PPI≥0.5组(42例)。测量所有研究对象24h动态血压、尿微量白蛋白。比较各组指标,分析PP、PPI与MAU,尿β2-MG之间的关系。结果 EH+DM组 24hPP、PPI、MAU,尿β2-MG明显高于EH组(P<0.05); 24hPP≥60mmHg组MAU、β2-MG水平明显高于24hPP<60mmHg组(P<0.05); PPI≥0.5组MAU,尿β2-MG水平明显高于PPI<0.5 组(P<0.05); PP、PPI与MAU、尿β2-MG成正相关。结论 PP、PPI是预测高血压合并2型糖尿病早期肾损害的良好指标。
Abstract:
Objective To investigate the association between Pulse Pressure(PP), Pulse Pressure Index(PPI)and Early Renal Injury in patients with hypertension complicated with type 2 diabetes mellitus. Methods 160 cases with essential hypertension were divided into the hypertension complicated with diabetes group(Complication group,n=78)and hypertension control group based on complication with or without diabetes(Control group,n=82). The complication group were divided into different groups according to the levels of ambulatory PP(APP)(<60mmHg or≥60mm Hg)and PPI(<0.5 or≥0.5). Urinary-MAU,β2-2MG and 24h ambulatory blood pressure levels were detected. Results PP,PPI,MAU and β2-MG in complication group were significantly higher than those in control group(P<0.05); MAU and β2-MG in 24hPP≥60mmHg group were significantly higher than those in 24hPP<60mmhg group(P<0.05); MAU and β2-MG in PPI≥0.5 group were significantly higher than those in PPI<0.5 group(P<0.05); The PP and PPI was positive correlated to the MAU and β2-2MG. Conclusion PP and PPI can sensitively predict the early renal impairment.
