摘要:
目的 探讨CD7+的急性髓系白血病抗原的表达情况及临床意义。方法 采用流式细胞仪和几组三色荧光标记抗体检测26例CD7+AML患者骨髓的免疫表型。结果 26例CD7+AML中,M1 10例、M2a 3例、M4a 2例、M5a 10例、M6 1例。CD33表达阳性率为100%,CD13为96.2%,CD15为76.9%,CD117为92.3%,CD11b为57.7%,MPO为84.6%,CD19为19.2%,CD22为11.5%, CD34为69.2%,HLA-DR为88.5%,其中同时表达CD34与HLA-DR抗原者占61.5%。结论 CD7抗原在AML各亚型中的表达率M1、M5a最多,未见M3表达,且CD7+AML多同时表达CD34、HLA-DR,其化疗效果差、完全缓解率低,免疫表型的检测对指导临床治疗与判断预后有重要的价值。
Abstract:
Objective To explore the clinical significance and immunophenotye in cases with CD7 positive acute myelocytic leukemia. Methods Immunophenotye of bone marrow in cases with FAB typed AML was analyzed by means of flow cytometry with 3-color fluorescence labeling antibodies. Results Of 26 cases with CD7 positive AMl, 10 were patients with M1, 3 were patients with M2a, 2 were patients with M4a, 10 were patients with M5a and 1 were patient with M6. The expression rates were 100% in CD33, 96.2% in CD13, 76.9% in CD15, 92.3% in CD117, 57.7% in CD11b, 84.6% in MPO, 19.2% in CD19, 11.5% in CD22. Also in 26 cases, 18 cases expressed CD34, 23 expressed HLA-DR and 16 cases(61.5%)expressed CD34 and HLA-DR at the same time. Conclusion CD7 positive AML is possibly an independent type originated from Hematopoietic stem cell at CD7+/CD34+ or earlier stage. Patients with CD7 positive AML have bad prognosis so that determination of immunophenotype may show important value in guiding treatment and identification of prognosis in patients with leukemia.
