摘要:
目的 探讨层连接蛋白受体、E-钙粘附蛋白和增殖细胞核抗原在胃癌中的表达及其 意 义。方法 对我院1994~1998年经病理诊断明确、随访资料完整的65例胃 癌组织标本、35例 癌旁组织,采用免疫组化法检测LN-R、E-cad和PCNA的表达情况,并分析其与胃癌临床特征 、预后间的关系。结果 胃癌中LN-R阳性表达率为38.5%,癌旁组织为9 .7%,二者间差异有统 计学意义(P=0.004); E-cad阳性表达率为72.3%,癌旁组织为94.3%,差异有统计 学意义 (P<0.01); PCNA胃癌中的阳性表达率为100%,癌旁组织为76.5%,差异有统计学 意义( P<0.001)。胃癌组织中LN-R阳性表达与肿瘤大小、大体形态、浸润深度、淋巴结转 移、临 床分期密切相关(P<0.05); 阳性表达者根治术后复发转移率及病死率均高于阴性者 (P< 0.05)。胃癌组织中E-cad阳性表达与淋巴结转移密切相关(P<0.05); 胃癌组织 中PCNA阳 性表达与临床各参数无关。结论 胃癌组织中存在着LN-R、PCNA蛋白高表 达、E-cad蛋白低表达现象,LN-R与胃癌侵袭转移的发生密切相关。
Abstract:
Objective To investigate the expression and Significance of La minin re ceptor(LN-R), E-cadherin(E-cad)and proliferation cell nuclear antigen(PCNA ) in patients with gastric carcinoma. Methods Sixty five Gastric carcinoma specim en and 35 adjacent tissues diagnosed by pathology and with follow-up data from 1 994~1998 were detected by immunohistochemical assay for the expression of LN-R, E-cad and PCNA. The results were analyzed. Results The positive rate of LN-R in the gastric carcinoma tissues was significantly higher than tha t in the adjacent noncancerous tissues(38.5% vs 9.7%)(P =0.004). The posi tive ra te of E-cad was 72.3% in gastric carcinoma tissues and 94.3% in the adjacent n on cancerous tissues(P<0.01). The positive rate of PCNA was 100% in gastri c carci noma tissues and 76.5% in adjacent noncancerous tissues(P<0.001).The po sitive e xpression of LN-R in gastric carcinoma tissues was associated with tumor size, m orph ological type of cancer, invasive depth, lymph node metastasis and TNM stage( P< 0.05). And the positive patients had higher recurrence, metastasis and mortali ty than negative ones after radical resection(P<0.05). The positive ex pression o f E-cad in gastric carcinoma was correlated to lymph node metastasis(P<0 .05). The positive expression of PCNA had no relation to clinical parameters. Conclu sion These results demonstrate that higher expression of LN-R and PCNA a nd lower expression of E-cad is existed in gastric carcinoma, and the expression of LN-R is correlated to the invasion and metastasis in gastric carcinoma.
