摘要:
目的 探讨256层iCT测量肝脏总体积及各肝段体积的应用价值并分析其与肝功能Child-Pugh分级的相关性。 方法 收集52例乙肝肝硬化患者和48例正常组资料进行对照,应用Philips 256层iCT专用软件分别测量肝脏总体积 及Ⅰ~Ⅷ段体积并做相应分组,分析其与Child-Pugh不同分级相应体积变化规律。结果 52例乙肝肝硬化组肝脏总 体积为(989.4±293.7)cm3,标准化后肝脏总体积(SLV)为(532.3±143.2)cm3·m-2,其中Child-Pugh A级 14例,B级16例,C级22例,Child评分为(8.71±2.80),SLV与Child评分呈负相关(r=-0.685,P<0.05),r2为 0.47。肝功能为Child-Pugh A、B级患者SⅠ~SⅢ体积和其占全肝体积比例均较正常组增大(P<0.05);肝功能 为Child-Pugh C级患者SⅠ~SⅢ体积较正常组减小(P<0.05),但其占全肝体积比例较正常组增大(P<0.05) ;Child-Pugh B、C级患者SⅤ~SⅧ体积和其占全肝体积比例均较正常组减小(P<0.05)。结论 256层iCT不仅可 以观察乙肝肝硬化后肝脏整体形态学变化规律,还可以较准确测量各肝段体积,并与肝功能Child评分有较好的相 关性。
Abstract:
Objective To investigate the measurement of the volume of the whole liver and each hepatic segment by using 256-slice intelligent CT and analyze the correlation between the volume of the whole liver and each hepatic segment and Child-Pugh classification. Methods By collecting and comparing the data of 52 patients with hepatitis B-related cirrhotic and 48 normal people,this research used Philips 256-slice iCT to measure the total liver volume and the volume of I ~ VIII segments and to make corresponding divisions,and finally analyzes its change rule between this application and different classifications of Child-Pugh on volume. Results The total liver volume of these 52 patients with hepatitis B cirrhosis was(989.4±293.7)cm3. Standardized liver volume( SLV) was(532.3±143.2)cm3·m-2. Among this,Child-Pugh A 14 people,B 16 people,C 22 people and the average Child score was(8.71±2.80). SLV was negatively correlated with the Child score(r=- 0.685,P<0.05) and a decision coefficient(r2) of 0.47. In other words,SLV decreased gradually with the rising of the Child score. Patients with liver function for Child-Pugh grade A and B SⅠ-S Ⅲ volume and its whole liver volume percentage increased in the normal group. The volume of SⅠ-S Ⅲ in the Child-Pugh C class was reduced,but the proportion of the total liver volume was larger than that of the normal group. The volume of SⅣ-SⅧ and the proportion of total liver volume in patients with hepatitis B cirrhosis were reduced. Therefore liver volume of liver cirrhosis was asymmetrical. Conclusion The morphology change rule of the liver with hepatitis B-related cirrhotic and the volume of each liver segment can be observed by 256-slice CT,which is better correlated with Child score.
