摘要:
目的 探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者肝肾功能的变化及临床意义。方法 选择OSAHS患者50例,根据睡眠呼吸暂停低通气指数(AHI)设立轻度、中度、重度组,选择健康者40例作为对照组。观察各组血清谷草转氨酶(AST)、谷丙转氨酶(ALT)、谷草/谷丙比(ST/LT)、碱性磷酸激酶(ALP)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL)、尿素氮(BUN)、肌酐(Scr)、尿素氮/肌酐(BUN/Scr)水平的变化,并分析与AHI、最低血氧饱和度(LSaO2)的相关性。结果 与正常对照组比较,轻、中、重度OSAHS组ALT、TC、AST/ALT、BUN/Scr水平均升高(P<0.05),中、重度OSAHS组的AST、LDL、Scr水平升高(P<0.05),重度OSAHS组TG、BUN水平升高(P<0.05)。OSAHS组各组间比较:与轻度OSAHS组比较,中、重度OSAHS组AST,ALT水平升高(P<0.05),重度OSAHS组TC、LDL、Scr水平升高(P<0.05); 与中度OSAHS组比较,重度OSAHS组TG水平升高(P<0.05)。OSAHS患者组肝、肾功能和脂代谢指标异常人数构成比高于正常组(P<0.05)。OSAHS组血清ALT、AST、TG、TC、LDL、Scr水平与AHI呈正相关(P<0.05),血清ALT、TG、Scr与LSaO2(P<0.05)呈负相关。结论 OSAHS可能引起患者一定程度肝脏功能和肾脏功能损害。
Abstract:
Objective To explore the effect of obstructive sleep apnea hypopnea syndrome(OSAHS)on tests of hepatic and renal functions. Methods According to sleep apnea hypopnea index(AHI), 50 patients with OSAHS were divided into mild, moderate and severe groups of OSAHS and 40 control subjects admitted into our hospital were recruited. The differences of the levels of biochemical examination AST, ALT, ST/LT, ALP, TG, TC, LDL, Scr, Bun and BUN/Scr concentrations among each group were analyzed. Also, the correlations between the apnea hypopnea index(AHI), lowest saturation of blood oxygen(LSaO2)were analyzed. Results The levels of ALT, TC, AST/ALT, BUN/Scr were significantly higher in groups with mild, moderate and severe OSAHS than those in the control group; The levels of AST, LDL, Scr were significantly higher in groups with moderate and severe OSAHS than those in control group; The levels of TG、BUN were significantly higher in groups with severe OSAHS than those in control group(P<0.05). The levels of ALT, AST were significantly higher in groups with moderate or severe OSAHS than those in the mild OSAHS group; The levels of TC, LDL, Scr were significantly higher in severe OSAHS group than those in the mild OSAHS group; The level of TG was significantly higher in severe OSAHS group than those in the moderate OSAHS group(P<0.05). ALT, AST, TG, TC, Scr were positively correlated with AHI(P<0.05), but ALT, TG, Scr were negatively correlated with LSaO2(P<0.05). Conclusion Results demonstrated that OSAHS had a certain extent association with liver and kidney damage.
